NCLEX-RN® Lab Values Guide: What to Memorize and How to Use Them
The lab values NCLEX® actually tests — normal ranges, critical thresholds, and how to spot the lab result that should change the nurse's next action.
Overview
Lab values appear on almost every NCLEX-RN® question that has a chart. You do not need to memorise hundreds of them — about 20 lab values cover roughly 90% of test-day items.
This guide gives you the short list, the easiest way to remember each one, and how the exam tests them.
The 20 lab values to memorise
How the NCLEX® tests labs
Three patterns repeat:
- "Which finding is the priority?" — pick the most life-threatening abnormal lab.
- "Which lab should the nurse report to the provider?" — same idea, often with a list of values.
- "Which intervention is best given this lab?" — match the abnormal value to the right action (e.g. K = 6.1 → hold K-sparing diuretic, prepare insulin + dextrose or Kayexalate, place on cardiac monitor).
The exam does not usually ask you to recite a normal value. It assumes you know the normal and tests what to do with the abnormal.
"Critical" labs — when to act now
Some lab values are dangerous enough that a competent nurse acts before reading the rest of the chart. Memorise these critical thresholds:
ABG interpretation in 30 seconds
Use the ROME mnemonic: Respiratory Opposite, Metabolic Equal.
- Look at pH first. < 7.35 = acidosis, > 7.45 = alkalosis.
- Look at CO₂. If pH and CO₂ move in opposite directions, the cause is respiratory.
- Look at HCO₃. If pH and HCO₃ move in the same direction, the cause is metabolic.
- If either CO₂ or HCO₃ is abnormal in the opposite direction needed to compensate, that compensation is partial.
A common test pattern:
pH 7.30, CO₂ 50, HCO₃ 25. → pH down, CO₂ up — opposite, so respiratory acidosis (uncompensated). Common cause: COPD, opioid overdose, hypoventilation.
Anion-gap and lactate cues
When lactate is high (≥ 2.0) in a clinical scenario with infection or low BP, think sepsis. When glucose is high with ketones in a chart for a type-1 diabetic, think DKA — and watch potassium carefully (apparent normal K with low total-body K).
Coagulation labs — match drug to test
- Heparin → aPTT (1.5–2.5× control, often 60–80 sec).
- Warfarin → PT / INR (target 2–3 for most, 2.5–3.5 for mechanical mitral valves).
- DOACs → no routine monitoring.
- Platelets are not a coag panel item — but always check before invasive procedures and after heparin (HIT).
How to study labs
- Three columns: Lab · Normal · "If abnormal, do X". Hand-write 5–10 a day until you've covered the 20.
- Drill weekly. Cover the "do X" column and recite it from memory for each lab.
- Always tag lab errors in the error log to the specific lab — patterns become obvious quickly.
FAQ
How many lab values do I need to memorise for the NCLEX®?
About 20 highest-yield values. The BMP, CBC, basic coag (PT/INR, aPTT), ABG components, and a few cardiac/inflammatory markers (troponin, BNP, lactate) cover most exam questions.
Are lab values different by lab or by reference source?
Slightly. Most NCLEX® questions either include the normal range in the question or use NCSBN's published reference ranges. Pick one mainstream nursing reference textbook and memorise its set — they are all close enough for the exam.
Do I need to know units?
Yes. Mix-ups between mg/dL and mEq/L cause errors on calculation items. Glucose is mg/dL. Sodium and potassium are mEq/L. Creatinine is mg/dL. Memorise the unit with the value.
What about pediatric or pregnancy lab values?
Know the adult ranges first. Pediatric and pregnancy variations appear in only a few specific items (e.g. higher WBC in newborns, dilutional anaemia in late pregnancy). Spend most of your study time on adult values.
What is the fastest way to remember normal values?
Spaced repetition with daily 5-minute drills for two weeks. Many candidates use a printed one-page chart taped near their study desk. The one-page redraw drill (cover the page and rewrite all 20 values) is the highest-yield single exercise.
